Serious Case Reviews

When a child dies, and abuse or neglect are known or suspected to be a factor in the death, local organisations first priority should be to consider immediately whether there are other children at risk of harm who require safeguarding. Then organisations should consider whether there are any lessons to be learned About the way in which local professionals and agencies work together to safeguard children; Reviews should identify clearly what these lessons are, how they will be acted upon, and what is expected to change as a result; and as a consequence, improve intra and inter-agency working and better safeguard children.

The PSCB Serious Case Review Panel (SCRP) oversees the work of the Review with members drawn from constituent agencies of the PSCB. The SCRP commissions an independent Overview Report. The author utilises all the Internal Management Review reports provided.
